There is a $7 fee (plus $2 to use a card) for each transcript request beginning July 1 after a student's senior year.

You must call the finance office and pay over the phone before the request is fulfilled.  PH. 801-798-4030

Due to privacy laws, transcripts may only be requested by the student or a parent/legal guardian when the student is under the age of 18.

If the student is over 18, the student must give authorization in this form and designate any other person to pick up the transcript.  The designated person must show a legal form of picture identification and sign when they pick up the transcript.
