Section 504 of the Rehabilitation Act of 1973 is a law that protects individuals who have a qualifying impairment from discrimination.
The purpose of Section 504 in the educations setting is to provide a student who has a qualifying impairment with an accommodation plan, when an accommodation is necessary for that student to have an equal opportunity to learn and to participate at school as compared to the average student of the same age and grade, who does not have an impairment.
Whether currently enrolled in Nebo School District (NSD) or enrolling for the first time, the parent/guardian must sign a consent form to start the evaluation process. This also applies to students enrolling in NSD who have a 504 plan that was written by another school district. When the consent form is signed and received a school has up to 45 school days to gather data such as staff/teacher input, medical notes, grades, attendance, conduct student observations, conduct testing, etc. and to make a determination.
For Students in 8th through 12th grade
The parent/guardian should contact a school administrator and ask about a 504 evaluation. Via email, an administrator will provide the parent/guardian with a link to a form titled, “Consent to Evaluate for Section 504.”
For Students in Pre-K through 7th grade
The parent/guardian should contact a school administrator and ask about a 504 evaluation. An administrator will provide the parent/guardian with a consent form via Infinite Campus, the district’s student information system.
